Some Champions can indeed be obtained through events, Fusion, or by every day-login but in the end, the strength of your team will depend on RNG which can be very frustrating. When we know that the chance of summoning the best Champions is 0.5% with Ancient and Void Shards or 6% with Sacred Shards, we conclude that it is almost impossible to plan the strategy of progress according to the Champion’s skill description. That basically means that players cannot plan their progression and that they are dependent on luck. Most of the Champions can be obtained only by summoning them and summon chances are similar like in other games. Players need to collect as many different Champions as possible and to use them in different game segments that awards them with various rewards needed for improving the Champions they collected. When we talk about the atmosphere in Raid Shadow Legends, we must emphasize that this game does not bring anything truly new to the Gacha mobile genre. It doesn’t bring anything truly new to the Gacha mobile genre… In the matter of the fact, Raid Shadow Legends MEME is one of the most googled keywords regarding this game.Īll that doesn’t change the fact that Raid Shadow Legends is one of the best Gacha games on the market and that it is so popular for the reason. Marketing of Raid Shadow Legends is so aggressive that some people started to make MEMEs about the game sarcastically and funnily, only because of constant commercials. One year after the release Raid Shadow Legends has more than 10 million downloads, it is one of the Top-grossing Gacha games on Google Play and its overall score is constantly rising (because of aggressive marketing approach, a lot of community reacted negatively when the game was released reviewing the game with the lowest score).Ĭurrent Metascore on Google Play is only 3.9 only because the community is divided concerning this game – people who actually play the game rate it with 5 while all others annoyed with constant commercials on all social media, rate it with 1. This is especially important when medications are listed in a separate Medication List without correlating information. Documentation of Medications Relating to Diagnoses: Notation of specific medications or other treatment relative to the diagnosis is considered adequate to demonstrate that a condition has been addressed during the visit, as long as the note specifies that the medication or treatment is associated with the specific diagnosis. The information described above does not need to be present in a particular portion of the medical record documentation and need not appear together in the same section or portion of the note. Any additional free-form text portion of the medical record. This documentation may be described as MEAT and should validate that the condition was Monitored, Evaluated, Assessed/Addressed and/or Treated.ĭiagnoses and the MEAT that support those diagnoses may be described in the patient’s: Evidence of any billed diagnosis codes should be described fully described in the medical record, except for status codes (see below). Monitored, Evaluated, Assessed/Addressed, Treated (MEAT): ICD-10-CM Official Coding Guidelines specify that all conditions should be coded and documented which affect patient care, treatment and management. Therefore, statements such as “same as last visit” or “see results from ” are not acceptable. A diagnosis may only be coded when it is explicitly named in the medical record.Įach encounter: Documentation of an encounter must be complete and not depend on reference to another encounter. CMS looks for a full description of the patient’s condition. If the information does not exist in the visit note, CMS considers that it did not happen. The existence of “history of” a diagnosis is not sufficient. If an ICD-10-CM code is billed by a provider, the following documentation must be present:ĭiagnosis by appropriate provider: Only a physician or other qualified and licensed provider legally accountable for establishing a patient’s diagnosis can “diagnose” a patient.Īll billed diagnoses must be documented: Written documentation to substantiate a billed diagnostic code must be included in the notes for the date of service associated with the claim. DOCUMENTATION OF BILLED DIAGNOSTIC CODESĪgeWell New York adheres to the ICD-10-CM Official Coding Guidelines, which are released an updated annually. In this case, the record must indicate that the record was signed electronically. An electronic signature is also acceptable. Provider name, credentials, and signature: Each encounter in the medical record should include a legible signature of the provider’s name and credentials (e.g.: MD, DO, NP, PA, etc.) The provider name and credentials may be pre-printed in a documentation form, in which case the form should be separately signed by the provider. Telehealth: Telehealth visits may be documented as a face-to-face visit only when the services are provided using an interactive audio and video telecommunications system that permits real-time interactive communication. Visit date: The medical record must include the date of the patient’s visit, including month, day and year. Alternatively, if the pages following the first page documenting a patient visit do not include patient identifiers, entries such as page number (coupled with visit date) may be used to ensure that a reviewer of the record can easily determine that the pages reference the same visit. Acceptable patient identifiers include patient’s first and last name along with either date of birth, account number or medical record number. Utilization of at least two patient identifiers is required. Patient identification on each page: Each page of the medical record should clearly identify the patient. Poor handwriting may be responsible for legibility issues, as are the use of acronyms that are not otherwise widely used by the medical community. While a digital or typed record is ideal, any handwritten entries in a medical record must be easily read. Legibility: All entries in the medical record must be legible. KEY COMPONENTS OF A COMPLIANT MEDICAL RECORD The following is being provided to ensure that all AgeWell New York providers are knowledgeable about what constitutes a compliant medical record and to provide the tools to support proper coding and documentation of diagnoses in the medical record. In addition, it is fundamental to ensuring compliance with CMS and NCQA billing guidelines. Complete and accurate documentation in the medical record is an essential part of quality patient care. The biggest potential issue is that SB->SE doesn't really allow you all the pre-Duel "buffing" time of using any/all abilities and items that you can to ensure you win, though it could allow a much shorter time to react, particularly if you just Duel straight from Invis (this is probably a really bad idea though). However if you're playing him as a fairly greedy Pos4, then a Blink can help you get some really good stuns or Echo Sabre Slows, and in theory, you could use it to set up a really long ranged Slow from Primal Spring by Blink->Tree Dance->Spring, since that's ~ 3000 units worth of range (most Blinks will be ~1000, even though max range is 1200, since you have to have a high degree of technical skill to hit consistent 1200 Blinks, and Dance+Spring have ~1000 unit range).įor LC I'd say it's more down to whether or not you can get a good won Duel with a Shadow Blade, vs using Blink. Monkey King would probably do better with a Shadow Blade->Silver Edge than a Blink Dagger if he's being played as a core- most likely a mid or offlaner. For others, it's all about the Shadow Blade- Slark, for example, has huge synergy with Shadow Blade due to the passive effects of his ult. In my plebeian opinion, Shadow Blade vs Blink Dagger in general is really down to "what are you going to get more mileage out of".įor some heroes, that's Blink far and away. Mod's let me know if I am breaking any rules. I do try to answer other peoples questions when I see them, I just don't know a lot. Sorry for posting a lot in this sub, its just hard to get answers from people in game. Why would you get it over a Blink Dagger? (THE IS AN EXAMPLE, DON'T JUST FOCUS ON SHADOW BLADE ON LC!) Is there something else to this?Ī good example of what I am talking about is Shadow Blade on Legion Commander. The only reason I can come up with is that higher MMR players think that the rest of us are to stupid to use Blink Dagger so they just say to get Shadow Blade instead. It's even in a few guides or high level players will recommend it. However in DotA 2 it seems to be built by everyone on everything (again probably more common at lower MMR's). Typically on heroes like Sniper, Drow, Kunkka (is that spelt right?) and so on. More commonly at lower MMR's/Skill levels. They were build fairly often on certain hero's. I played a bunch of DotA 1 and HoN where Lothar's Edge and Assassin's Shroud were items. /r/noobsoftheancient - Play with and against other new players.Learndota on Mumble Learndota on Steam Learndota on Discord Related Subreddits Reports are NOT for people you disagree with. There is the new "Lightspeed Wireless" which sounds a bit like Razer's Adaptive Frequency Technology (AFT) but doesn't go quite so far in its claims. In fact, the G903 is the same as the G900 save for a few minor differences. The PMW3366 optical sensor is highly regarded in the gaming community, and that same technology is found in the G903. The Logitech G900 is considered by many to be one of the best wireless gaming mice on the market. Look ma, no hands Compared to the Logitech G900 and POWERPLAY charging There is also a switch to turn the mouse on or off but no compartment for the 2.4GHz wireless dongle, which is a bit weak.
